We provide high quality care to Participants living with a disability in the community. Our nursing and supports team are qualified, experienced and long term.
Nursing
We prioritize the well-being of our participants through a dedicated team of highly qualified registered and enrolled nurses. Our nurses oversee all tasks performed by our Support Work team. Registered nurses provide a comprehensive array of essential services, including wound management, continence care, feeding assistance, medication management, ventilation support, and mental health services. Each participant is assigned a dedicated nurse who acts as a central point of contact and care coordinator. This nurse develops personalized care plans based on individual needs and goals, closely monitors progress, collaborates with participants and their families to establish measurable outcomes, and strives diligently to achieve optimal well-being.
Personal Care and Capacity Building
Our qualified and experienced Support Workers offer extensive support to our Participants in daily activities including personal care, medication compliance, household tasks and implementing programs designed by the therapy team. Additionally, we emphasize the development of essential skills such as budgeting, meal planning, shopping, household management, and Centrelink administration. We are focused on helping our Participants to operate as independently as possible.
Community Supports
We offer transportation and assistance to facilitate safe community engagement for our Participants. This support extends to various activities such as shopping trips, social gatherings with family or friends, as well as attending medical or employment appointments. Our dedicated team is readily available to provide the necessary assistance throughout these activities.
